I've noticed many, many runs where one (or more) of the bosses' loot will be Free-For-All with no loot roll at all. This has only occurred when we queue with multiple people from our guild/server.
My current best guess is that it's a glitch involving the loot settings of the multiple people that queue and/or the dungeon leader. Thinking this may be the case because my wife and I are always running around on Free-For-All loot for dailies and don't think to change it when queue'ing for a dungeon.
