@decx: I only have one account with 2 DID numbers. Not sure what a VM is...
@mm: My settings are exactly the same as posted (apart from using the chicago.voip.ms server) and I am running the latest firmware (1.3 build 2744).
The only thing I can think of now is that the number I have going to voicemail was the first one to be configured in voip.ms and the second number (my home phone) was ported from bell and configured a couple weeks later. Perhaps the MWI/VMWI "signal" is coming from the first number instead of the second one (or should both numbers MWI/VMWI work at the same time?).
