Fixed.

And the fix turned out to be a power cycle to the DSL modem.
(modem and power strip are under my couch, so cycling power either requires quite a bit of work, or flipping the breaker)

Upload speed is still 0.5 - 0.6 Mbps, but my DL speed is now slightly over 2 Mbps.
