Do I need to license App Patrol to do bandwidth limiting/QoS on the USG50?

No, you don't need App Patrol license. See BWM configuration page and embedded BWM help on how to configure.

If I just want to make sure my Sprint Airave Femtocell (VoIP App) gets priority I just need to check the "Enable Highest Bandwidth Priority for SIP Traffic" Box?

I found the SIP setting to be entirely ineffective; we still had QOS issues and I eventually had to abandon the SIP trunks for a PRI.

Well, there is more to it then just enabling the check box.
- the BWM is managed for egress traffic only
- you need to set proper egress speed for your WAN interface first in order for any BWM to be effective

Interesting. And disturbing. I actually called ZyXEL U.S. support about it and was told, very specifically, "Check the box. That's all there is to do and all you can do."

That said, I did set the inbound and outbound speeds as well. We really, really need to get to a point where we have across the board QoS for things, like SIP/VoIP, that need it.

Yes, checking the box and setting the WAN interface speeds should be enough for SIP only. For other services you need to do it manually as per my earlier example thread.
