Can't imagine the impact of every SB getting a firmware refresh at the same time! Or does each CMTS just works its way down the list of SBs attached to that CMTS?

The upgrade statement is in the bootfile, when your device resets for any reason the device pulls the new bootfile. This is called the "attrition method", nice and slow. But everyone will get the new bootfile in time, sometimes it takes about two or three months.
