[Help Me] VOIP QOS Setup for D-Link 2640B Router

I have a D-Link 2640B router. I would like to setup the QOS for my VOIP. I have a CISCO PAP2t NA as my ATA. In the QOS section, I am required to specify a value for the following parameters:

1) Traffic Class Name
2) Assign ATM Transmit Priority
3) Mark IP Precedence
4) Mark IP Type Of Service
5) Mark 802.1p if 802.1q is enabled on WAN

For Set 1, I need to adjust the following parameters:

1) Physical LAN Port
2) Protocol
3) Source IP Address
4) Source Subnet Mask
5) UDP/TCP Source Port (port or port:port)
6) Destination IP Address
7) Destination Subnet Mask
8) UDP/TCP Destination Port (port or port:port)

For Set 2, I need to adjust the following parameters:

802.1p Priority

Assume my IP address for my CISCO PAP2T ATA is 192.168.1.5.

Could someone help me to adjust the above parameters.

1) Physical LAN Port - This might be asking which physical LAN port in back the cable is connected too, 1, 2, 3, or 4?
2) Protocol - Should be TCP/UDP or Both or Any?
3) Source IP Address - Should be the range or specific IP address of the device on your network, i.e. PC or other.
4) Source Subnet Mask - Might be the same as the routers Sub Net Mask. Not to sure about this.
5) UDP/TCP Source Port (port or port:port) - Should be from 0 to 65535 and anything in between.
6) Destination IP Address - Should be from 0.0.0.0 to 255.255.255.254
7) Destination Subnet Mask - Should be from 0.0.0.0 to 255.255.255.254
8) UDP/TCP Destination Port (port or port:port) - Should be from 0 to 65535 and anything in between.

I would call D-Link support and ask then exactly how this is configured.
