Yes, tried resetting the phone modem, my internet modem and my router and it continues to persist when it happens unfortunately.

Maybe a faulty splitter? Are both modems fed off 1 splitter?
I just took a look and there is 3 splitters down where everything goes, my internet and phone are off of 2 different splitters but both of those splitters connect which makes me wonder.
I think there is to much splitter action going on, as they all seem to feed into each other in some manner. I guess I need to try and find a good splitter with lots of attachments lol
