How to interpret this Event Report from CenturyLink?

I received the following Event Report from CenturyLink. It has since been resolved. As far as I can tell it never affected me. I'm just curious if there is a way to determine which card/port my service comes from?

I never heard of them sending any reports to customers before. Where did you get that report?

CenturyLink Event Reporting Information Console (ERIC). Just sign up and you'll get reports that sometimes have relevance.

I've just never seen them be so specific as to the issue, "Card 13 ports 9-16 no surf". As a result, I'd like to know, in the future, if they go to such detail, if it will affect me.
