The replacement cost if you lose a CableCard is $100, which is conveniently what UPS insures every package for by default. "Handling" is just code for "extra profit" since you've already got those people working the warehouse sending and receiving huge volumes of other equipment; CableCards are a drop in the bucket. Verizon already charges more than twice the industry average for CableCards, so it's clearly just there to further discourage their use.

For other people that have this same question, try calling support. I ordered a Cable Card through them, and I asked if there was any way to waive the shipping charge. It took a little convincing, but eventually I was able to get them to agree that $20 for shipping something that fits inside an envelope was too much. They weren't able to waive it up front, but they notated my account for a credit after I received it. When I called back to get the credit, there was no hassle.
