I like the DSLExtreme approach

They block it by default. Why have it open for viruses to exploit unsuspecting people and spammers if it's not needed by a large majority of customers? But if I want it for any reason, it can be unblocked by filling out a quick web form. DSLExtreme does not forbid servers in its AUP so I'm comfortable that they'll continue to support my ability to unblock that port.

I also don't really have a big problem with port 25 blocks as long as the ISP's server accepts any FROM address. But from a service that supports servers I do, of course, want the ability to unblock.
