Mutualphone (outbound only)

None of these lock equipment. (NOTE: I believe that with BroadVoice you must specify "generic SIP" instead of your actual device to avoid them locking equipment, at least if you have a Sipura device.)

There are other providers that are geared toward Asterisk/IAX users rather than single-line users with SIP devices that I didn't mention.
