I would try to do it in steps:

1) factory reset 4200 modem and leave it as-is.
2) factory reset your d-link
3) disconnect it from your 4200
4) Login to the d-link and do the full setup (PPPoE username/password, static DHCP, port-forwards)
5) After applying the changes connect the modem again and make sure your PC is talking to the ROUTER (IP rang 192.168.0.X)

If your IP range is 192.168.2.X then something is wrong.
