Qualifying and expiration date?

Some questions:

1) I assume it must be an annual contract. What do you have to sign up for to get the $200 gift card. Is it just the 10/2 speed or must you have a higher speed? Is there a smaller amount gift card for 10/2 service?

2) When does this end? Specifically, is it available after 12/31/06?
