What the OP is describing are ILEC boundaries. These came about over 100 years ago when a company setup shop in an area.

While one county in any direction you can get broadband throughout the entire county. My farm is 500 yards from the county border and I cannot get broadband because I'm in BellSouth territory while my neighbor enjoys broadband because the next county is in Verizon/Windstream territory.
