Like CylonRed said it can vary based on many different factors. But as an example, I'm at 9435 feet out of a Verizon CO in Lynnwood, WA. and have 6.0/768 package. I sync at 6/768 and my last speed test was 5178/660. I have not had the line tested for ADSL2. Never had a problem with speeds or packet loss.

I was a bit over 10,000 feet when Verizon added a coil to my line and I had the 6/768 package and got about 5200/685 average so it depends on alot.

Same-ish here. About 9500 ft from the CO, somewhat decent loop, on 6.0/768 package, and get about 5.0/640 on average.
