Florida unemployment compensation

If anyone here has received a 1099-G recently from the state of Florida for the purposes of income taxes, I could really use some information off of the form.

I'm doing taxes for a relative and know how much unemployment income they have received, but I need stuff like payer's name, address, city, state, zip code, phone number, and especially the payer's ID.

If anyone knows this information, could they please message me privately with the information? Thanks.
