Tape backup fail, with Symantec backup exc 11d

So lately I been getting this error below when it trys to do a backup, right now i am realyl stuck... I even changed the cleaning tape... Anyone have any idea?
Mods, please move to correct forum if this one is incorrect.

Label Library 00040 -- A tape read/write error has occurred. This is usually caused by dirty read/write heads in the tape drive. Clean the tape drive, and then try the job again. If the problem persists, try a different tape. You may also need to check for problems with cables, termination, or other hardware issues.

How old are your tapes? How many uses are on the tapes? Are you using a single tape over and over, or are you cycling them? Can you read from other tapes?

At first glance, this looks like a bad tape (or set of tapes) to me or worst case, a failing tape drive.

Edit: Yes..I see that he did mention doing the tape cleaning..

What tape drive is it? LTO? DAT? Which one by what manufacturer?

Well, without troubleshooting you're not going to get it working. You can't jump from step A to step Z. Troubleshooting takes time and patience, so you need to slow down ...

Try reading from an old tape. If you can't, my guess would be that the tape drive is bad. Was BackupExec installed with the Symantec tape drivers, or did you install it using the native tape drivers? BackupExec should have presented you with this option when it was installed.

If it was the native tape drivers, stop all the BackupExec processes and open the Windows Backup program. The drive should show up there ... attempt a small backup. Does it work? If not, you have a tape drive problem and need to contact the manufacturer for service.

Don't know how much usage your drive gets, but moving parts do fail. We've had to replace drives regularly, and the occasional drive controller, too.

We had this issue, and (for us) it wasn't related to a cleaning tape problem.

We had to contact Symantec, and they suggested to update all firmware on the tape drive (autoloader) and also update the drivers for that device.

I would go into detail, but I didn't handle the issue.
