A CV tech and supervisor showed up to do my install this morning. It took about 90 minutes from start to finish, about a half hour of which was spent waiting for a call back from dispatch. They replaced the modem and installed a new ground on the tap outside my house, but didn't replace any other equipment or cabling. I do not have Optimum Voice. Equipment is a pfSense router/firewall and gigabit Ethernet end-to-end.
Speed tests (apparently the CV speed test server in NY can't keep up!)

I'm using Newshosting. 11.5MB/s is the norm for me and I've never seen go above 12.0 even for a few seconds. But there's a difference between what Newsleecher says and what pfSense says. When pfSense is showing as high as 103 MBps inbound, Newsleecher shows 94MBps. TCP overhead accounts for maybe half of that difference, but I'm not sure what the rest might be. Also you can see that it's using nearly 1mbps upstream just for the ACK packets! These two graphs were made at the same time (ignore the glitch in the pfSense chart):
