Populate word form with excel data

Is there any way to populate a word form (using form fields) with excel data?

I'm tracking pending, active, and completed cellular cases using an excel spreadsheet broken down by year. I'd like to take one row of data and populate the cells that contain data to a word document that contains my examiner's notes.

Use a "mail merge".

Download these sample documents and save them in the same folder... should work well enough.
