Is there any way to populate a word form (using form fields) with excel data?

I'm tracking pending, active, and completed cellular cases using an excel spreadsheet broken down by year. I'd like to take one row of data and populate the cells that contain data to a word document that contains my examiner's notes.

Thanks. Was hoping somebody had done it before but I'll venture done one of Google's paths on my own. Hopefully I'll get it to work.

Use a "mail merge".

Download these sample documents and save them in the same folder... should work well enough.
