58dbmv are high transmit levels usually caused by modem going trough to many splitters if you are hooked up straight into main line coming into the house and you forward levels are good -2.2
more likely a network issue consider a "low return" problem in the network line that you are hooked up to....

tech that comes out has to escalate the service call for a lineman or network department to come out and trouble shoot the problem most of the time is the network amplifier.. easy and fast to fix

Nothing is "easy and fast" with Comcast. I had a similar problem that went on for 3 months. Finally a call to the Comcast executive complaint center got someone out that was able to determine the problem was in the box in my alley. That same day I found out FIOS was available in my neighborhood. I made the switch and have not looked back.

I asked about my options, one was Saturday and they never mentioned after-work hours. Since I was not available Saturday...

My setup goes through two splitters (not optimal, but my setup requires it). The problem was a very low signal coming from the tap (as I had suspected) and the tech was able to make a call and get the signal pumped up a notch.

Even though Comcast has been (almost)rock solid ever since my 9/11/06 issues, I'm getting tired of the yearly "we need to raise your prices" whine... If only I had more broadband choices that didn't suck...
