DNSguy pointed out to me the other day apparently we do have a FAQ on the RDNS delegations. here it is:

I am not sure if the prices are still correct but I think they might be on track. I think you might still have to call them and some have advised that calling was quite a bit quicker, none the less they may need that information on that form as well.
