Not to make any excuses "but:" many of the problems we're all seeing here are typical of fast growth, which has obviously exceeded supply capability.
There is never an excuse, in my mind, for mis-billing people. That's just Un-Sat! and regardless of what anyone ever tells you, a reversal is a very simple task.
As to the problem(s) with speed, if anyone goes through the NY switch, there are some MAJOR problems there and Phoenix is trying to address them. There have also been problems in Tampa, FL this week as well.
