Few observations:
1) Most PhonePower reviews seem good. Read the individual reviews and scrutinize the reputation of the reviewer (oldtimer has more street cred than someone who is first time poster, etc)

www.extremefax.com/5) You have fairly high demands, but want the lowest possible price. Realize that you may not be attractive as a customer. The best providers want customers with lower maintenance demands, but who are willing to pay a little more for quality and reliability.
6) You mentioned Ooma. Yesterday the website Woot had refurbished Ooma Telo for about $ 145 including shipping. Fees would be about $ 3 or $ 4 a month. If you keep looking you could probably find this again.
7) I wonder if you realize that with most regular POTS phone companies, AND with most VoIP providers, pressing *67 does NOT block your outbound caller ID number when you are calling toll free numbers.
8) You could sign up with a VoIP provider to make outbound calls and not even transmit a standard outbound CID. For that matter, you don't even have to get a DID number from the company to begin with. One example: FlowRoute. Of course, if you don't transmit a standard outbound CID then other people may block YOUR calls.
