Definitely replace the fan - Noctua, Bitfenix, and Yate Loon have been recommended here by others. I have two Noctuas that have been running 24/7 since 2008, and two Yate Loons under the same conditions since 2007. Look for one that's 1300 RPM or lower (probably not lower than 1000). Once you do that, you can decide whether to leave the case fan as-is and flip the heatsink fan around, or leave that situated how it is now and having the rear case fan blowing in.

If you have no other case fans, you may want to do the latter. That will get the case air pressure close to neutral, if not positive. Positive air pressure is good as it helps keep dust buildup to a minimum.

Actually I would not recommend the S12 for radiators and heatsinks. It was designed as a case fan so has less static pressure than other models.
