Yes. They said they'd send the accountholder a claim form, but they haven't yet, to my knowledge.

Turns out they were never going to send a claim form and the live chat rep lied. Went to Cox on Euclid on Monday, and a supervisor said that self-installation of boxes waives Cox of all liability for damage because "there's no way to prove it" (or something along those lines). Gave them back the Mini Box that did it (SN: CS0008204421 SN2: CTBFDVNJZ), and got a new one.
