Does IIS have any logging? That log should tell you whether the server "sees" any login attempts. If it does then the problem isn't the forwarding but some setting on your server. Could be for example: anonymous login not allowed or non-existing directory associated with user.

I don't think it's anything to do with the PC as it works fine when connected directly to the cable modem. (ie without the router) - I also don't THINK it's anything to do with ports as I put the FTP server in DMZ on the router and still didn't work.
