Important Reminder: below is the Web Certificate Request Process - this is the only method to request a Web Certificate. This process, attached below, must be followed to ensure the Web Certificate will be issued to the customer in a timely and accurate manner.
Important Update: the attached Web Certificate Request form has been updated to distinguish between the $25 Web Certificate the $50 Web Certificate. The date the customer ordered FastAccess service will determine which Web Certificate the customer is eligible for:
Order service on or after May 12 $50 Web Certificate
Order service prior to May $25 Web Certificate
Web Certificate Request Process
On occasion, qualified Web Certificate customers have not received a Web Certificate because it was lost, never received, or customer did not qualify because the customer could not complete their service order on-line. A new process has been created to address these issues.
To request a Web Certificate for a qualified customer who has not yet received it, simply complete the Web Certificate Request attachment.
Steps to remember when requesting a Web Certificate:
1. Provide complete information on the Web Certificate Request form; this information includes:
New - select if it is the $25 or $50 Web Certificate
note number of times request made
your name
customer's name
customer's e-mail address
customer's DSL number
reason for the request
2. Once the required customer information has been typed onto the attachment, follow the steps provided on the attachment of how to send it; the steps include:
click File from menu bar
click Sent To
click Mail Recipient (as Attachment)
verify the e-mail's To field auto populates with: modemcredit@bellsouth.com
3. Ensure the Subject line on the e-mail is: Web Certificate Request; it should auto populate but if it fails to, please type in the appropriate title.
